In Provet, you can upload and use fillable PDF and Word forms for various purposes, such as patient, client, consultation, discharge notes and patient referral feedback forms during consultations. Forms can also be used for specific needs, such as dental charts, digital anaesthetic charts, consent forms, and health and vaccination certificates.

It is no longer possible to create new forms using the Build form function. If you have created forms using the old form builder tool, your forms are still available and you can edit them. While Word forms are still available, it is recommended that you replace them with PDF forms.
Note
You must use a version of Microsoft Word that supports the DOCX file format. Word for web is not supported. Additionally, versions of Microsoft Word meant for Apple computers (using macOS) cannot be used.
To ensure access to your forms for use outside of Provet, save them to an external location when you upload them, as forms cannot be downloaded from within Provet and are solely for in-system use once uploaded.
