Use Discharge Forms

Discharge forms are used to provide clients with detailed instructions and information following a consultation. These forms can be attached to other written instructions and emailed directly to the client during or after the consultation.

  1. Go to the Discharge instructions section on a consultation page.

  2. Select a form from the Add form drop-down list.

  3. The form dialogue opens. Select Open to edit the form.

  4. Provet autosaves the possible changes you make to the form. For other available options, see Use PDF Forms or Use Word forms.

  5. After closing the preview, the form appears under Forms on the consultation page (located just below the Discharge instructions section).

  6. To edit the form again, click the form title.
